Gathering Documents

Now that you’re ready to get going, you’ll need paperwork and a lot of it! Below is a list of documentation to gather before attempting to file your MA income tax return.

Copies of the previous year’s federal and state returns

Personal information for yourself, spouse, and dependents:

  • Legal name
  • Birth dates
  • Social security numbers

Record of all earnings. This includes W-2 forms from each employer and any 1099-MISC forms you’ve received from contracted work hours.

Record of earned interest and dividends from investments or banks. These can include 1099-INT, 1099-DIV, etc.

Proof of health coverage (1099-HC, 1095-B, etc.)

Records of paid school tuition and student loan interest paid (1098-T)

Record and total of child care expenses with provider information:

  • Establishment Name
  • Employer ID (EIN)
  •  Social security number (if private care)

Records of received Social Security benefits (SSA-1099)

Record of received pensions or annuities (1099-R)

Unemployment wage statements (1099-G)

Total rent paid with landlord or agent’s name and address

Records of medical expenses (cancelled checks or receipts)

Mortgage interest (1040) and property tax paid

Receipts from chartable donations with organization information

  • Name
  • Address
  • EIN

Bank account and routing numbers for direct deposit of a refund
